再発肝細胞癌に対する治療

再発癌であっても初発と同様の基準に従って治療を行うことが多い。

第一選択は肝臓移植あるいは外科的切除であるが、ミラノ基準(3個以内で各々が3㎝以下または単発で5㎝以下) を満たさない場合や、癌の性状、肝硬変、患者の社会的背景により困難な場合、幕内基準を満たさない、切除後の肝臓機能が問題になる症例や切除不能な範囲での再発症例には以下の治療法を選択する。

根治的肝切除後の５年累積再発率は77~100%と非常に高く、術後の死亡原因は肝機能不全(9%) ではなく癌の再発が大部分(82.5%)である[6]。再発率の低い治療法、早期再発発見、再発後の有効な治療法が重要となる[1]。

肝臓移植は、移植後の肝炎の再発のリスクファクターが、ドナーの年齢が50以上、ドナー入院期間が5日以上、レシピエントの年齢が40以上、レシピエントのMELD score(1) が27以上、という結果が示されており[4]、最も有効であると考えられている移植による治療であっても、その効果は大きく異なる。再発時に移植を選択するべきかは、以上のことを加味して決定する必要がある。

[6]では、生存率に関連するファクタがmacro vascular infiltrationであることが示されており、これが原因となって手術適用でなくなることが多く、根治的治療が不可能となる。このことからも外科的切除が最も適した治療法といえる。また、予後因子として、肝硬変、慢性活動性肝炎(CAH)、HCVが挙げられており、癌自体の特徴や形状等はあまり関係していないことがわかったことから、再発癌に対しては癌のみの治療でなく再発する肝臓という土壌や環境(肝硬変等)に対する治療が必要であると考えられる。

[3]では、再発肝内癌に対するRFAが、初発癌に対して切除術を施行した症例に対して、生存率の点から有効であるとしている。但し、予後が良いのは、肝内単発の再発であり、肝内多発の再発や肝外での再発については依然として予後は良くなく、これらのタイプの再発に対する有効な治療法は必要であることが述べられており、最近になって発展してきたRFAによる治療はすべての再発に効くわけではない。

[2]では、再発後二回目の肝切除術後の予後を決める因子として門脈／静脈／胆管浸潤がない(P＜0.01)、混合型(P＜0.01)、腫瘍径3cm以下(P=0.02)、癌の発達型(P=0.02)、線維性皮膜の存在(P=0.03)が挙げられている。また、全体の生存率を決める因子として、二回目の切除時の門脈浸潤がない(P=0.01)、初発時に単発である(P=0.01)、再発まで一年以上である(P=0.02)、の三つが示されている。

以上のことから、再発肝細胞癌に対しては、可能な限り外科的切除や移植が望ましいが、 RFA等の治療が良いと主張する施設もあり、治療別RCTが行われていない現状では実際の成績は不明である。

2. Journaling filesystems:

Imagine writing stuff on a lot of different notes and pieces of papers, etc.,
and then suddenly getting hit in the head and forgetting everything (call this
rebooting). You suddenly don't know which was the note you were in the middle
of writing, and you may end up finding a note saying "kill <name>" not knowing
you really meant to write "kill <name>'s jobs on the department's workstation,
because they are hogging all resources" before you got hit on the head. That's
why you should have a journal. Write everything that you do in there, one
entry after another, and only when you complete a whole note, cut it out of
the journal and keep it.

Also, when you go to the bathroom, don't forget to write down in the journal
about whether you're already done with #1, #2, or #3 (don't ask what #3
is...). That way, if you suddenly get hit on the head (say, the nice fake
plant over the toilet falls on you) you won't get embaraced, asking yourself
questions like "Oops, I don't remember if I did #2 or not, so should I reach
for some toilet paper or not?" If you had a journal, everything would have
been much simpler. Just look in there, and see what you've been up to.
